Learn about FCH's Theory of Change

As part of the Foundation for Community Health's strategic planning process, we developed a PDF icon theory of change.  In it, we describe our plan to pursue FCH's mission and vision to make an impact on the communities we serve through three priority areas:  Strengthen Organizations, Improve Community Assets, and Support Local Decision-making.

Foundation for Community Health

We work together with people and organizations to improve the health and well-being of residents in our community, especially those that have historically been under-resourced.

The Foundation for Community Health was initially funded with the net assets of the sale and conversion of Sharon Hospital in 2003 and is a supporting organization of the Berkshire Taconic Community Foundation, Community Foundations of the Hudson Valley and Northwest Connecticut Community Foundation.
