GRANTEE PROFILE: HRHCare Community Health Center

HRHCare Community Health is a system of nonprofit, New York State licensed, Federally Qualified Health Centers (FQHCs). Founded in 1975, the HRHCare network includes 26 health centers throughout the Hudson Valley and Long Island. HRHCare operates three Health Centers in Eastern Dutchess County at Amenia, Dover Plains, and Pine Plains.  These centers provide primary and preventive treatment including adult and pediatric care; obstetrical and gynecological care; family planning; HIV counseling, testing, and referral; dental services; mental health counseling; nutrition counseling; podiatry; WIC services; health education; and a full complement of outreach, translation, transportation, benefits counseling and health insurance enrollment assistance. Recently the Amenia center was pleased to welcome Pediatric Nurse Practitioner Patty Boltin and Dentist Dr. Susanna Jacob to the team.   HRHCare works to improve community health through partnerships and collaborations with local community leaders, businesses, social service agencies, housing organizations, food banks, and agricultural worker alliances.

Currently, HRHCare is undertaking the Eastern Dutchess School-Based Preventive Dental Initiative, an FCH funded project. The goal of this program is to continue to improve health outcomes and dental hygiene in school-aged children in the Dover Union Free School District, engaging 90% of elementary and middle school students in education on the importance of proper oral health through in-class and school-wide presentations. This program also aims to engage at least 50% of eligible elementary students in the school-based preventive dental screening and sealant program.
