Grant Program Profile: Community Health & Wellness Center

Community Health & Wellness (CHWC) received a grant for $1,333,265 from the Foundation for Community Health (FCH) toward the total cost of $5,400,000 to establish a Federally Qualified Community Health Center (FQHC) in North Canaan, Connecticut.  These funds cover Phase I costs related to the purchase of the property on East Main Street, preliminary building expenses, the hiring of an engineer and architect, demolition of existing structures and costs associated with fees and permits and consultants to assist with applications and determining the feasibility of additional funding streams to assist with Phase II construction.

 Phase II will include the construction of a 7,200 sq. ft., two-story building to house primary care and mental health services and space for integrating community support services, such as a community room for various services including addressing social determinants of health, adding the convenience of optimizing services under one roof and bringing much needed, affordable quality healthcare services to the North Canaan region. 

When completed, the new and only FQHC in Northwest CT will provide quality integrated Primary Care and Behavioral Health services in one location for residents of North Canaan and its 11 surrounding towns, who currently need to travel to either New York or Massachusetts to access comprehensive, affordable health care. The availability of affordable, high-quality medical services will serve as a support to local businesses in attracting and retaining workers and contribute positively to the area’s economic growth by adding jobs. 

Additionally, CHWC has been the beneficiary of other FCH grants that have helped the health center throughout the years.  The funding has helped CHWC with services such as to establish the dental clinic at the Torrington site, conduct oral health training for nursing home staff and for screening and isolation procedures for COVID-19 patients. 

 CHWC is an award-winning FQHC that provides high-quality integrated medical, behavioral health, and specialty care to patients, without regard for their ability to pay, using a multi-sector approach to strengthen overall community and population health.
