Prescription Assistance Fund
Welcome to the Foundation for Community Health Prescription Assistance Fund. This program helps to pay for prescriptions, premiums for prescription drug insurance, and/or prescription co-pays.
In order to be eligible for this program, an individual must be at or below 300% of the Federal Poverty Level. Once approved, the individual will be covered initially for three months. During that period:
- If the individual is 65 or older and eligible for State Aid (e.g. EPIC in NY), they must apply for this aid within those first 3 months, otherwise they will no longer be eligible for this program.
- If the individual is turning 65 this year, they must apply for Medicare Part D. The individual can apply three months before or three months after the month of their birthday. If the individual is already 65, they will be covered for the rest of the year, but they must apply for Medicare Part D during the open enrollment period in order to have coverage in the following year.
- If the individual is at or below 150% of the Federal Poverty Level, they must apply for Medicaid and receive either a denial or proof of an impossible spend-down (the amount of money they are required to pay before they receive aid) in order to remain eligible for this program.
The Foundation offers this program by working with the following agencies: Northwest Hills Council of Governments in conjunction with its respective towns’ selectmen and social workers, Dutchess County Community Action Agency, and Columbia County Healthcare Consortium.
